Data will help with lessen fraud and showcase those with social determinant issues. But the last mile isn’t more data acquisition; it is patient clarity, confidence, and motivation. It is understanding what the next steps you need to take in your care plan, how to take them, whose going to pay for them, and remain focused on valuing and investing in the your health. We need to answer the questions what, why, and how for the patient.
