In the last few years, we have seen new care models emerging. Some care models focus on populations, specialization, and setting. As we watch the models unfold, we see the spectrum move toward:
- High touch management, especially for the complex chronic care needs
- Providing more access or convenient for healthier populations
- Creating more aligned models that reduce the total cost of care
- Redesign clinical workflows to create more efficiency of staff, care rendered, and care sought
We note entities such as Landmark, Oak Street, ChenMed, 98point6,and Bright.md continuing to reframe care and care delivery. We, of course, see the additional of more in-home care and virtual care which we dive deeply into in an upcoming blog where we will explore activation, utilization, and assurance of a quality experience.
