We see the market really moving to identify, embrace, and create an informational hub around social determinants. We see health plans creating a social infrastructure with Aunt Bertha or entities like Healthify or Wider Circle to support their members needs. We watch as these strategies move from high risk support to outreach and engaging the “rising risk” members and patient to ensure they understand the resources available to them. These strategies do drive down cost and create a happier, healthier patient.
